Summary

  • We are seeking an RN to provide personalized care and support to caregivers and their loved ones. No night shifts or weekends required, and no on-call duties! As an RN Care Manager, you'll assess needs, create individualized care plans, conduct home visits, and ensure exceptional consumer care.

  • Will Do: Conduct home visits in assigned territory to assess home safety, gather changes in health status, and provide clinical coaching Following an established care plan, provide ongoing coaching and support to caregivers Document and review records for accuracy and timeliness, ensuring compliance with regulatory and accreditation standards Perform other duties as assigned.

  • Bring: Must hold an active Registered Nurse (RN) License in MA Minimum two years of recent experience in direct care of elders or adults with disabilities Ability to travel within assigned geographic area; valid driver’s license and automobile insurance Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a health care setting Additional state-specific qualifications may be required. Requirements

Candidates must hold an active Registered Nurse (RN) license in Massachusetts and have at least two years of recent experience in direct care for elders or adults with disabilities. A valid driver's license and automobile insurance are required for travel within the assigned geographic area.
