The Hospice Admission Nurse is responsible for admitting patients to the hospice service and palliative care service and developing appropriate plans of care. The hospice admission nurse is responsible for accurate and efficient completion of all admission components, including HIS and comprehensive assessment and establishment of the initial plan of care in keeping with best practice standards in addition to the development of the OASIS data set and orders for service for palliative care.
Education: BSN preferred
Knowledge of State Regulations, Medicare, and Medicaid regulations is required. Experience in home health or acute care setting
Certificates, Licenses, Registrations: Current Connecticut Registered Nurse License
Possess reliable transportation.
