About the role

The Resident Assessment Nurse conducts the RAI process to identify resident needs and develop comprehensive care plans. They collaborate with the interdisciplinary team and manage clinical documentation to ensure accurate reimbursement and high-quality care.

Description THIS POSITION IS LOCATED IN CHILLICOTHE, MO

The Resident Assessment Nurse is responsible for conducting the Resident Assessment Instrument (RAI) process to ensure individual resident medical, psychosocial, emotional and physical needs are identified, and care plans are developed to assist residents in achieving their highest practicable level of function. The RAN is part of the facility team. The Administrator expects the RAN to be a visible, engaged member of the building, attending standing meetings, contributing to IDT discussions, and meeting building expectations on conduct and professionalism. The Resident Assessment Nurse is responsible for conducting the Resident Assessment Instrument (RAI) process to ensure individual resident medical, psychosocial, emotional and physical needs are identified, and care plans are developed to assist residents in achieving their highest practicable level of function.

The role requires physical presence in the building and direct contact with residents, families, and the interdisciplinary team.
  • Conduct required resident interviews (BIMS, PHQ-9, customary routines, pain assessment, interviews).
  • Perform Section GG functional observations and Section K nutritional assessments.
  • Gather, review, and synthesize supporting documentation across nursing, therapy, social services, and dietary.
  • Develop and maintain Care Area Assessments (CAAs), comprehensive care plans, and work in conjunction with the nursing team to ensure current resident care plans.
  • Participate in daily clinical huddles, IDT meetings, care conferences, Risk meetings and family meetings, when necessary.
  • Coordinate with floor nursing on documentation needs that support accurate assessments.
  • Hand off completed clinical documentation to the central Resident Assessment Coordinator for technical coding and transmission.

Requirements

Qualifications, Skills and Requirements Experienced LPN with a minimum one-year nursing experience in a skilled nursing/long-term care facility. Registered Nurse (strongly preferred). Prefer Resident Assessment Coordinator (RAC) Certification. Excellent verbal and written communication skills with residents, families, all levels of staff and external customers. Proficient computer skills essential including the ability to use Point Click Care and conduct online submissions.? Must possess the ability to work harmoniously with and supervise other personnel. Must possess a high degree of personal accountability, responsibility and the ability to make independent decisions when circumstances warrant such action. Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, and procedures, etc., that are necessary for providing comprehensive activities to residents.
