Position Overview As a Licensed Practical Nurse (LPN), you will play a key role in delivering high-quality, resident-centered care at an Assisted Living and Memory Care community.
You'll collaborate closely with caregivers, leadership, and healthcare providers to ensure safe, compassionate, and personalized care.
Key Responsibilities Provide direct nursing care, including medication administration, treatments, and assessments Monitor resident health and communicate changes to the care team Assist in developing and updating individualized care plans Support residents with memory loss through patience, empathy, and engagement Build strong relationships with residents, families, and team members Maintain accurate and timely clinical documentation Mentor and support caregivers and new staff Ensure compliance with state regulations and company policies
Required: Graduate of an accredited practical nursing program Active Minnesota LPN license in good standing Preferred (not required): Assisted Living, Memory Care, or Long-Term Care experience We are happy to train the right candidate with a passion for senior care.
