Deliver Patient-Centered Care – Conduct assessments, provide treatments, and ensure strong follow-up care. Collaborate Across Teams – Partner with providers and healthcare staff to create effective care plans. Educate & Empower – Support patients and families with knowledge on conditions, treatments, and prevention. Administer Medications – Safely deliver medications and treatments per protocols. Maintain Records – Accurately document patient care and changes in condition. Drive Quality Improvement – Participate in initiatives that enhance care and outcomes. Required: Nursing degree from an accredited nursing program Current LPN or RN license in state of practice Valid driver’s license
Preferred: 2 yrs of experience of nursing experience 2 yrs of experience in primary care
