Job Description

Under the Direction of the program Director, the transitional care specialist is accountable for facilitating, implementing, and confirming postacute transition plans for CoxHealth’s patient population. The position will respond to Hospital and community-based service requests from the nursing staff, the case managers, the social service workers, hospitalists, or private attending physicians practices.

Education

  • Required: High school diploma or equivalent

  • One year hospital and/or healthcare experience
  • Skills:
  • Good communication skills – Ability to communicate effectively in written format and/or oral presentations
  • Strong analytical & problem solving skills; semi-independent in decision making
  • Ability to maintain organization in a changing environment
  • Exhibit initiative, responsibility & flexibility
  • Must be able to initiate and understand research related to projects
  • Learn and appropriately utilize all required computer applications, including but not limited to E-mail, Windows-based or Microsoft programs
  • Ability to write effective documentation for processes and procedures.
  • Ability to make routine decisions in accordance with departmental policies and procedures.

Requirements

A high school diploma is required, while an associate degree and one year of healthcare experience are preferred. Candidates must possess strong communication, analytical, and computer skills.
