You, our Ideal Candidate As a valued member of our in-house team, not a contracted service, you play a vital role in supporting our residents’ recovery and independence. You’re compassionate, collaborative, and skilled in guiding individuals through customized rehabilitation plans. You’re flexible, empathetic, and bring a positive attitude that helps residents feel encouraged and empowered.

Responsibilities

  • Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this position:
  • Performs and documents an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention. Identifies and documents goals, anticipated progress, and plans for reevaluation.
  • Plans, prepares, or carries out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
  • Records prognosis, treatment, response, and progress in patient's chart or enters information into computer.
  • Evaluates effects of treatment at various stages and adjusts treatments to achieve maximum benefit.
  • Confers with the patient, medical practitioners, or other appropriate parties to plan, implement, or assess the intervention program.
  • Complies with professional, ethical, and legal standards.

Required Education and Experience • Education

  • Graduate of an accredited physical therapy program
  • Experience: At least 2 years' experience as a physical therapist
  • Licenses/Certifications: Current Physical Therapist state license
