We seek a skilled field Licensed Vocational Nurse (LVN) to join our amazing and expanding team! As an LVN with Crossroads, you will provide care and support to patients during times of illness or injury.
In this role, the ideal candidate will have knowledge and skill in supporting patients as they recover from acute illness. This includes direct patient care, educating families, providing care aligned with patient’s values, and providing support for patients and families. We seek a compassionate nurse with excellent interpersonal skills who possesses a thorough understanding of home health care.

Must be a graduate of an accredited LVN program with a current California license, CPR certification, and at least one year of experience in home health or hospice. A valid driver's license, reliable transportation, and the ability to pass a background check are required.
