Job Description

SUMMARY OF POSITION Functions as staff LPN utilizing the nursing process with performing comprehensive psychiatric /mental health assessments. Evaluates mental status and refers clients to other treatment providers when necessary. Assumes primary responsibility and accountability for ones nursing practices as defined in the state specific Code of Nursing Practice, as it pertains to a licensed practical nurse and functions under the supervision of an RN or physician. he Columbia clinic is seeking a team-oriented LPN to join a team of therapists, nurse practitioners, psychiatrists, case managers, and support staff to deliver care that saves lives. Mental health experiences preferred. At least two years' experience in the field as an LPN. This is an office-based job. Hours of Monday through Friday 8-5 and is not telehealth or is not a work from home position. ESSENTIAL DUTIES & RESPONSIBILITIES Performs comprehensive psychiatric /mental health assessments, evaluates mental status and refers to other treatment providers when necessary. Monitors changes in health status, including but not limited to vital signs, weights, nutritional status and disease states. Administers and monitors medications as ordered. Monitors medication effectiveness, side effects and compliance. May perform assessments for any side effects/EPS. Provides phone triage and addresses medication issues with consumer when necessary. Provides proper education and guidance as necessary. Assists patients with accessing medications including obtaining prior authorizations and accessing prescription assistance programs. Provides healthcare education to clients and/or families or staff as assigned. Provides CLIA Waived laboratory testing as ordered. Maintains CLIA waived laboratory environment including Quality Control testing and documentation. Monitors received laboratory values and reports abnormal values as required. Phlebotomy may be required. Provides employee health services as directed, including annual influenza vaccination clinics and staff TB skin testing. Works in collaboration with other members of the treatment team and in collaboration with external health care providers as necessary. Nurses may provide direct physical supervision to Certified Medical Assistants and delegate tasks in accordance with the Tennessee Nurse Practice Act and other applicable regulations. Completes clinical documentation within organization’s policy standards. Maintains all mandatory trainings up to date. May provide assigned residential nursing services as directed. KNOWLEDGE, SKILLS & ABILITIES Must have the ability to work cooperatively with all staff and physicians and function as a valuable member of the treatment team.

Qualifications

Education Level Graduation from an accredited nursing program is required. Years of Experience Minimum of six months to one year experience in psychiatric or equivalent mental health related nursing experience is preferred. Certification/Licensure Licensed Practical Nurse (LPN). Requirements

Requires graduation from an accredited nursing program and a valid Licensed Practical Nurse (LPN) license. At least two years of experience as an LPN is requested, with a preference for experience in psychiatric or mental health nursing.
