Make a lasting difference in the lives of patients and families Supportive, collaborative team environment Competitive compensation Tuition reimbursement for eligible full-time employees Free continuing education opportunities for all employees Career advancement and professional development opportunities Comprehensive medical, dental, and vision insurance 401(k) with employer match Paid time off, paid holidays, and family & pet bereavement leave Pet insurance As an Admission Nurse, you will: Complete comprehensive admission assessments to evaluate patients' physical, emotional, psychosocial, and environmental needs Develop and initiate individualized plans of care in collaboration with physicians and the interdisciplinary care team Perform initial nursing evaluations and ongoing reassessments as needed Accurately document patient assessments, clinical findings, and care plans in the electronic medical record Communicate changes in patient condition promptly with physicians, Clinical Team Managers, and other members of the care team Ensure compliance with HIPAA regulations, company policies, and clinical standards Identify safety concerns and report significant findings to appropriate leadership Promote continuity of care while supporting positive patient outcomes Perform additional responsibilities as assigned to support patient care and branch operations

Requirements

Graduate of an accredited nursing program Current, unrestricted Registered Nurse (RN) license in the state of employment Minimum of one (1) year of clinical nursing experience Ability and willingness to travel throughout the assigned service area (approximately 50%) Reliable transportation, valid driver's license, and current auto insurance meeting state requirements Ability to lift, push, pull, and support up to 50 pounds, including assisting with patient transfers and moving equipment

Preferred Qualifications
