The Care Team Hospice is seeking a compassionate and dedicated Medical Social Worker (MSW) to join our growing team. This is an opportunity to make a meaningful difference by providing emotional support, counseling, advocacy, and resource coordination for patients and families navigating the hospice journey. As a Hospice Medical Social Worker, you'll work closely with an experienced interdisciplinary team to address the emotional, psychosocial, and practical needs of patients and their loved ones. You'll help families navigate difficult decisions, connect them with valuable community resources, and ensure they receive the support they need every step of the way. As our patient census continues to grow, this part-time opportunity has the potential to become a full-time position.

  • Conduct comprehensive psychosocial assessments to identify patient and family needs while developing individualized plans of care.
  • Provide emotional support, counseling, crisis intervention, and guidance to patients, families, and caregivers throughout the hospice journey.
  • Assist with advance care planning, community resource coordination, financial assistance referrals, and bereavement support.
  • Collaborate with physicians, nurses, chaplains, and the interdisciplinary team to ensure coordinated, patient-centered care.
  • Maintain accurate, timely documentation of assessments, interventions, and care coordination activities within the electronic medical record (Homecare Homebase).

Qualifications

  • Master's Degree in Social Work (MSW) from an accredited program preferred.
  • Previous experience in hospice, home health, healthcare, or case management preferred.
  • Knowledge of psychosocial assessment, crisis intervention, grief counseling, and community resources. Field Role Requirements
  • Valid driver's license, reliable transportation, and current automobile insurance.
  • Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening.
  • Current TB testing in accordance with agency/State policy.
  • Current CPR certification as required for the role.

Requirements

A Master's Degree in Social Work is preferred, along with experience in hospice, home health, or case management. Candidates must possess a valid driver's license, reliable transportation, and current CPR certification.
