As a Personal Care RN, here’s a big-picture view of what you’ll do

  • Conduct initial admissions, comprehensive assessments, and routine reassessments of patients receiving personal care services
  • Develop, implement, and update individualized Plans of Care in collaboration with physicians, patients, and caregivers
  • Perform supervisory visits with home care aides to ensure care is delivered in accordance with the Plan of Care and agency standards
  • Provide ongoing clinical oversight, guidance, and education to personal care aides and caregivers
  • Evaluate aide competency and document supervisory visits per regulatory requirements
  • Monitor patient condition and promptly communicate changes to physicians and interdisciplinary team members
  • Provide skilled nursing services as needed and ensure all care aligns with physician orders and state regulations
  • Educate patients and families on disease processes, safety, and care needs
  • Ensure accurate, timely, and compliant documentation of all visits, care plans, and communications
  • Coordinate services and collaborate with internal teams to ensure continuity and quality of care
  • Participate in case conferences, quality assurance activities, and performance improvement initiatives
  • Assist with care transitions and discharge planning as appropriate

A few must-haves for PCSS Registered Nurses

  • Graduate of an accredited nursing program and active RN license in Virginia
  • Minimum of 2 years of RN experience or completion of a preceptor program
  • Active CPR certification (adult, child, and infant)
  • Knowledge of personal care services regulations and supervisory requirements preferred
  • Strong clinical judgment, communication, and organizational skills
  • Ability to manage multiple patients and priorities independently
  • Valid Driver’s License and reliable transportation
