About the role

Manage patients enrolled in Remote Patient Monitoring services, focusing on blood pressure and weight monitoring. Coordinate enrollment, education, and documentation to ensure compliance with CMS requirements and organizational policies.

Description Responsible for managing patients enrolled in Remote Patient Monitoring (RPM) services, with a focus on blood pressure and weight monitoring programs.

Coordinates patient enrollment, education, and ongoing engagement to ensure compliance with Centers for Medicare & Medicaid Services (CMS) requirements and organizational policies.

Schedules patient appointments through the Electronic Health Record (EHR), conducts routine follow-up calls, documents patient interactions, and collaborates closely with providers, billing, and clinical teams to support quality patient care.

Oversees RPM equipment inventory, coordinates equipment distribution and recovery, and ensures timely documentation and billing processes while maintaining compliance with CMS guidelines.

Performs additional administrative and patient care coordination duties as assigned to support the success of the RPM program.

Responsibilities

Manages patients enrolled in remote monitoring (RPM) services including blood pressure and weight modalities Schedules patient appointments using the Electronic Health Record Enrolls patients in the RPM program and educates on proper use of clinic-provided equipment Communicates with enrolled patients via phone to meet Centers for Medicare & Medicaid Services (CMS) billable criteria for RPM services Conducts follow up calls and manages documentation of care with enrolled patients per organization policies and guidelines Collaborates with provider teams to manage care of enrolled patients Manages equipment inventory for RPM services Coordinates with team members and leader(s) to ensure appropriate inventory of equipment; identifies equipment needs in a timely manner Coordinates with leader(s) and the billing department to publish charges for unreturned equipment Ensures documentation needs are met to effectively bill for RPM services Coordinates with Billing department to maintain CMS criteria is being met and practices are current Completes documentation in the EHR in a timely manner to ensure claims are accessible Other duties as assigned

Requirements

Education and Qualifications Certified Medical Assistant (CMA) or Licensed Practical Nurse (LPN) or Registered Nurse (RN) Maintains Basic Life Support (BLS) If RN, maintains Advanced Cardiovascular Life Support (ACLS) Job knowledge and skills Cardiology experience of at least 5 years for CMA Cardiology experience of at least 1 year for LPN or RN Moderate computer skills to include, but not limited to, use of Electronic Health Records system, use of Excel Spreadsheets, and web-based portal(s) Communicating with compassion, both within the clinic and with patients/families Critical thinking and prioritization of tasks Works well in a team environment Full-Time Benefits & Perks Set scheduled 4-day work week Comprehensive Medical, Dental, and Vision insurance (effective 1st of the month following 60 days) Company-paid Long-Term Disability (60%) and $25,000 Basic Life Insurance 6 Paid Holidays: New Year’s Day, Memorial Day, 4 of July, Labor Day, Thanksgiving, & Christmas PTO accrued at 3.08 hours per paycheck (80 hrs/yr) 401k matching program after 1 year worked VIP Status at both Southeastern Cardiology and Southeastern Gastroenterology Discounts through Uniforms of America, and the Working Advantage program.
