About the role

The Clinical Case Manager serves as the primary point of contact for clients, developing personalized treatment plans and facilitating individual and group sessions. They are responsible for managing a caseload of up to 12 clients, coordinating aftercare, and intervening in crisis situations.

What You’ll Do

Serve as the primary point of contact for clients during treatment, ensuring continuity of care from intake through discharge.

Develop personalized treatment plans, identify key problem areas, and collaborate with clinical staff to ensure goals are measurable and achievable.

Facilitate individual and group sessions using Evidence-Based Practices.

Coordinate aftercare plans and make referrals to support long-term recovery.

Manage a caseload of up to 12 clients with regular documentation and treatment planning sessions.

Intervene in crisis situations and manage client transitions across levels of care.

Maintain active participation in clinical team meetings, quality assurance efforts, and ongoing training.

What You Bring

A Bachelor’s degree in a behavioral health or related field (Master’s preferred).

Clear background screening and compliance with federal, state, and company policies.

Valid driver’s license with insurable driving history.

Current CPR/First Aid certification and TB clearance.

Excellent written and verbal communication skills.

Knowledge of clinical and medical terminology related to behavioral health.

Proficiency with Microsoft Office, Adobe, and G Suite applications.

Requirements
