About the role

The RN Case Manager is responsible for conducting initial patient evaluations and regularly reevaluating nursing needs to initiate and revise care plans. They provide specialized nursing services and maintain timely clinical progress notes and care conference summaries.

What you’ll be doing

  • Make the initial evaluation visit and regularly reevaluate the patient’s nursing needs. Meet Daily Productivity Standards. Initiate the plan of care and necessary revisions. Provide those services requiring substantial specialized nursing skills. Initiate appropriate preventative and rehabilitative nursing procedures. Prepare clinical and progress notes for each patient visit and summaries of care conferences on their patients in a timely manner.

  • What we’re looking for: A passion to serve and help others live their best lives possible. Graduate of an accredited school of professional nursing. Currently licensed as a registered nurse in the state of the job posting. Minimum of one (1) year of nursing experience in a home health setting. Must have current BLS Certification. Access to reliable transportation.

Schedule: Day shift Monday to Friday 8am-5pm Weekends as needed

Requirements

Candidates must be a graduate of an accredited professional nursing school and hold a current registered nurse license in the state of employment. A minimum of one year of home health nursing experience and a current BLS certification are required.
