Provide one-on-one personalized care to patients in their homes, assisting with activities of daily living and personal care. Responsibilities include monitoring vital signs, meal preparation, and maintaining detailed health records.

We are looking for a compassionate HHA or CNA in Espanola, NM to administer patient care in accordance with a physician established care plan.

This role is a 1099 contract opportunity, with care being provided at the patient’s residence.

Once our patients are accepted into the program, there is no expiration for this benefit, which allows our team of caregivers to build lasting relationships with their patients for years to come.

This patient has the following schedule openings: 7 hours a week, on Frida mornings starting at 8am or 9am.

You’ll get to advocate for the patients and work one-on-one to provide them with the care that they need.

This role will collaborate with CNA’s, HHA’s, Case Managers, and the patient’s physicians.

Offers are contingent upon passing a background check.

Responsibilities

Work with the patient in their home to provide one-on-one service in a comfortable and familiar setting. Assist with the patients’ activities of daily living as well as personal care. Light housekeeping Meal preparation Monitor vitals (vital signs, temperature, respiration, etc) according to plan of care. Remind patient of to take medication as prescribed. Maintain records and documentation of services. Communication with the patients’ family, doctors, and other prescribed individuals regarding the health and care of the patient.
