She/He applies specialized knowledge of pharmacotherapy to direct patient care, teaching, and consultation. Participates as a contributing member of ambulatory care teams such as rheumatology, dermatology, neurology, infectious disease, oncology, nephrology, or pulmonology. Provides clinical, pharmacy, and medication expertise in each area including pharmacy operations, prior authorization support and medication access, and medication therapy outcomes. EDUCATION: Doctor of Pharmacy and an ASHP accredited residency program or equivalent experience preferred. Licensed pharmacist in the State of Vermont. EXPERIENCE: 3 years of advanced practice experience in their area of expertise preferred or equivalent experience in ambulatory care or a clinical outpatient setting.
