The Case Manager develops and implements patient care plans while coordinating the continuum of care for individuals in an opioid treatment program. They act as a liaison between insurance providers and the treatment team to ensure optimum resource utilization.
Your Education, Skills, and Qualifications: Graduation from an accredited school of nursing. Minimum 2+ years’ experience in nursing practice Supervisory experience preferred, unless otherwise required by State or Licensing requirements. Prefer experience in behavioral health or addiction recovery field environment (inpatient or outpatient) Training or experience treating patients with MAT (medication-assisted treatment) is highly preferred There must be no history of alcohol or other drug misuse for a period of three years before employment Licenses/Certifications: Current LPN/LVN license within the state of New Hampshire Current CPR certification where required by state regulation.
