About the role

Provide professional housing support services to individuals with disabilities experiencing homelessness to help them secure and maintain independent living. This includes assisting with benefit applications, teaching basic living skills, and coordinating care with community providers.

  • Starting Pay Range: $24.59-$26.32 per hour Office Benefit Package: $50/Month subject to change The Housing Case Manager provides professional, quality program services to the clients served by the ReEntry House Housing Support Program (HSP) in Hennepin County. The Housing Case Manager will provide support to individuals with disabilities experiencing homelessness to help them live independently in the community. They will help clients locate, secure, and maintain independent housing in Hennepin County; provide instruction and monitoring of basic living and social skills to vulnerable adults to restore a client’s skills essential for managing his or her illness, treatment, and the requirements of everyday independent living.

  • Friday, 8 am - 4 pm or 9 am - 5 pm, 40 hours per week
  • Location: This positon is a hybrid model. Some work is done in the community with clients and other work can be done from home/in office.

Duties and Responsibilities

  • Provide Housing Support Services to eligible clients that are in compliance with County, State and Medicaid requirements. Using person-centered approaches, assist clients in obtaining and/or maintain housing, moving, and getting necessary furnishings. Assist clients with applying, maintaining, and understanding benefits, such as Cash, SNAP, HSS eligibility, medical insurance, and Social Security. Help with completing necessary paperwork, obtaining proofs, getting documents signed by Qualified Professionals, contacting relevant County, setting up appointments, and referring to outside resources when necessary. Provide necessary assistance in budgeting and use of Housing Support funds as required by DHS. Assist, teach, and support clients with activities of basic independent living skills, being a good tenant, and their rights as a renter. Find and assist clients with accessing community resources. Communicate effectively, professionally, and collaboratively with property management, county financial workers, other providers, and others in the community. Build professional relationships to find and maintain housing, help advocate for clients; provide property management-tenant support and mediation. Maintain records of progress for all direct and collateral contact. Documentation will include an assessment of client’s ability to function and manage the symptoms of his/her illness based on observation and interaction with the client. Assist clients to create a plan to work towards service goals in regular meetings with the client. Provide instruction and monitoring on the identified service areas indicated in the client’s Professional Statement of Need and/or Housing Focused Person-Centered Plan, and other collateral information. Continually monitor client progress and with the service team and modify the service plan as appropriate. Housing Case Manager will communicate changes in service needs and recommendations to Program Director/Mental Health Professional, Case Manager, and rest of service team on an ongoing basis to coordinate client care. Schedule appointments to see clients in their home or in the community to give requested services for the amount of time and frequency agreed upon by the service team and client and adjust as needed based on the client’s immediate needs. Intervene as necessary for emergencies and/or when he/she assesses that the client is unable to manage his/her illness in a reasonable manner. Contact appropriate emergency services, doctors, staff, and emergency contacts will be made to alert them of intervention planned or taken. Coordinate and cooperate with other agency and community providers to provide the best possible care for the client, including but not limited to landlords and other community resources. The Housing Case Manager will complete all necessary release of information documents for communication with other providers and keep it current for each client. Maintain compliance with applicable Minnesota Statutes or rules, including but not limited to DHS Maltreatment of Vulnerable Adults, Duty to Warn, Minnesota Data Practices Act, and HIPAA. Meet regularly with Program Director for individual and group reviews of client needs and receive direction for treatment. Must be able to transport clients in the community. Candidate will have a Bachelor's degree in Psychology or Social Work plus 2,000 hours work experience providing services to adults diagnosed with a serious mental illness
  • OR- No college degree and 4,000 hours supervised work experience in the delivery of services to adults or children with mental illness. Requirements

Requires a Bachelor's degree in Psychology or Social Work with 2,000 hours of experience, or 4,000 hours of supervised experience without a degree. Candidates must possess a valid driver's license, auto insurance, and a personal vehicle for transporting clients.
