About the role

Collaborate with an interdisciplinary team to develop and implement patient treatment plans. Provide direct patient care, perform evaluations, and deliver education to patients and their families.

Duties and Responsibilities

In this role you will be

An active member of the interdisciplinary treatment team and work collaboratively in the development, implementation, and revision of each patient’s interdisciplinary treatment plan.

Observing and evaluating each patient as well as documenting their observations and evaluations.

Providing the RN with relevant clinical information.

Using nursing knowledge of medical, surgical, and behavioral health nursing to effectively provide direct patient care, observing, and documenting outcomes

Providing patient and family education.

Working collaboratively as a member of the interdisciplinary team.

Qualifications

Required

  • EDUCATION: Graduate of an accredited LPN program
  • EXPERIENCE: 1 year of medical surgical experience preferred
  • Licensure/Certification:
  • BLS required or obtained within 1 month of hire
  • Current license as a Licensed Practical Nurse in Pennsylvania

Shifts: Rotation

Requirements

Must be a graduate of an accredited LPN program with a current Pennsylvania license. BLS certification is required, and one year of medical-surgical experience is preferred.
