Emplify Health by Gundersen is looking for a full-time Medication Reconciliation Tech.
The Medication Reconciliation Tech supports the pharmacy department by collecting accurate medication histories, identifying barriers to medication adherence, and assisting with medication reconciliation and safe transitions of care.
This role requires strong communication, problem-solving, and motivational interviewing skills, along with the ability to work collaboratively and support high standards of patient care.
