The Case Manager supports patients throughout their treatment episode by coordinating care, completing biopsychosocial assessments, managing FMLA and disability documentation, and assisting with discharge and aftercare planning. This role also oversees care for FADAP patients and serves as a key liaison among patients, employers, monitoring agencies, healthcare providers, and community resources to ensure continuity of care and positive treatment outcomes.
  • QualificationsEducation and
  • Experience: Master's degree preferred. High school diploma or GED required CPR, CPI and First Aid required within 30 days of hire 1-2 years' experience in behavioral health We are a Drug Free Company. Requirements

Candidates must have a high school diploma or GED and 1-2 years of experience in behavioral health. CPR, CPI, and First Aid certifications are required within 30 days of hire, with a Master's degree preferred.
