This role is responsible for supporting patients and families through assessments, care coordination, advocacy, education, and community resource management to promote overall well-being and quality of life. Minimum Requirements: Current Florida Licensed Social Worker (LSW) credentials required Knowledge of laws, regulations, and guidelines related to Senior Living and Home Health Minimum one year of experience in a healthcare setting preferred Knowledge, Skills & Abilities: Strong organizational and communication skills Detail-oriented with the ability to maintain accuracy daily Excellent interpersonal skills with a professional and compassionate approach Proficient in Microsoft Office programs including Excel, Outlook, and Word Ability to learn and use proprietary software as needed
