Job Description

In partnership with the health services team, the Caregiver is responsible for assisting the resident in all areas of care as assigned by the Director of Assisted Living, or designee. Essential

Duties and Responsibilities

  • Adheres to and conveys the SRG philosophy of person-centered care and Genuine Hospitality.
  • Maintains a professional appearance, manner, and approach while on duty.
  • Wears name tag, uniform, and adheres to SRG cell phone policy.
  • Reports to work on time and as scheduled per community policies and procedures
  • Receives report from the previous shift, reads Care Staff Assignment and End of Shift report for information and changes before providing resident care.
  • Documents care given via CareStream; adhering to policies and procedures.
  • Provides report to oncoming shift and documents on the End of Shift Report and CareStream before going off shift.
  • Completes incident reports when needed.
  • Sign out and carry equipment such as radio, tablet, and keys at onset of shift.
  • Answers resident call lights, door, and resident alarms promptly.
  • Provides resident assistance with activities of daily living, encouraging, and promoting residents’ self-care to the extent possible as needed or assigned.
  • Encourages and assists residents in individual and group program activities.
  • Assist residents with meals, housekeeping, and laundry services as needed, requested or assigned.
  • Maintains a clean, neat, comfortable, safe environment for residents, staff, and visitors.
  • Provides high quality resident care and recommends ways to improve quality.
  • Observes for changes in resident physical or mental status and communicates to the Director of Assisted Living, LVN/LPN, and/or manager on duty.
  • Responds promptly to residents, staff, or visitor emergencies, follows policies and procedures.
  • Demonstrates knowledge of and follows policies and procedures for infection control.
  • Familiar with fire, missing resident, and other emergency procedures; follows community policy and procedures detailed in the fire and disaster plans.
  • Maintain appropriate levels of confidentiality with resident and family information.
  • Attend and participate in training and team meetings as assigned.
  • Other duties as assigned

Qualifications

Required Skills/AbilitiesEducation/Experience/Physical Demands: Good physical and mental health Basic reading and writing skills and ability to speak English Good communication skills. Able to promote a positive working environment and encourage teamwork Good interpersonal and customer service skills. Good organizational skills and attention to detail. Experience with Yardi, eMAR, Schedule Pop, ADP, and/or other relevant platforms. C.N.A. or experience as a caregiver. High school diploma or GED First Aid / CPR certification required Ability to stand, walk, bend, and squat for prolonged periods. Ability to push and pull objects, lift, and carry up to 25 pounds, unassisted, on a frequent basis (occasionally up to 50 pounds, unassisted). Full use of hands, arms, and legs (for washing, bathing, assisting in feeding, and dressing residents, writing notations, operating vacuum cleaners, light housekeeping in resident rooms and bathrooms, etc.) While performing the duties of this job, the employee is occasionally exposed to wet and/or humid conditions, moving mechanical parts, fumes, airborne particles, outside weather conditions, and risk of electrical shock. The noise level in the work environment is usually moderate. While performing the duties of this job, the employee may occasionally be exposed to body fluids, infection, odors, and behavior of residents.

Requirements

Candidates must have a high school diploma or GED, First Aid/CPR certification, and experience as a caregiver or a C.N.A. Physical ability to lift up to 50 pounds and stand for prolonged periods is required.
