As a Registered Nurse, you will provide comprehensive nursing services to clients and patients receiving care through primary care, behavioral health, substance use treatment, outpatient services, residential programs, crisis services, and community-based settings. You will perform nursing assessments, coordinate care, administer medications, monitor patient health status, provide education, and work closely with physicians, advanced practice providers, and clinical teams to support whole-person care and positive treatment outcomes. nursing assessments, screenings, health histories, and vital sign collection to identify patient needs and support treatment planning · Provide direct nursing care and ongoing health monitoring for individuals receiving primary care, behavioral health, and substance use treatment services · Collaborate with physicians, advanced practice providers, pharmacists, and multidisciplinary treatment teams to coordinate patient care and improve health outcomes · Develop and implement individualized action plans that address both physical and behavioral health needs · Educate patients and guardians on medications, treatment recommendations, disease prevention, risk reduction, wellness strategies, and healthy lifestyle practices · Administer and oversee medications, therapeutic injections, and other nursing interventions in accordance with regulatory standards and program requirements · Monitor medication effectiveness, side effects, treatment compliance, and laboratory results while communicating findings with providers and treatment teams · Coordinate medication services with pharmacies, patients, families, and assistance programs to ensure continuity of care · Arrange and coordinate medical appointments, specialty referrals, special dietary needs, and follow-up care services · Assist physicians and advanced practice providers during examinations and clinical procedures · Collect laboratory specimens, perform requested procedures, and support diagnostic and treatment activities · Monitor infection control practices, patient hygiene concerns, and overall health and safety within assigned programs · Respond appropriately to medical concerns, urgent health issues, and emergency situations · Maintain accurate, timely, and compliant clinical documentation in accordance with organizational policies and regulatory requirements · Support organizational goals and participate in additional duties as assigned by program leadership Education, Experience, and/or Credential Qualifications: · Current Registered Nurse (RN) license in the state of practice · Willingness to obtain nursing licensure in additional states as needed · Ability and willingness to provide telehealth services to individuals served · BCLS certification required for FQHC services · ACLS certification preferred · Thorough knowledge of nursing principles, practices, and standards related to primary care, mental health, and substance use treatment · Knowledge of healthcare regulations, treatment protocols, and program-specific policies and procedures Additional Qualifications: · Valid driver's license, acceptable driving record, and current automobile insurance required · Successful completion of background screenings, including criminal history, driving record, abuse/neglect, and fingerprint checks · Strong verbal and written communication skills with the ability to communicate effectively and therapeutically with patients, families, providers, and community partners · Ability to maintain confidentiality and exercise sound clinical judgment · Demonstrated ability to manage multiple priorities while maintaining accuracy and attention to detail · Proficiency with computers, electronic health records, and related software applications · Ability to establish and maintain positive working relationships with patients, families, coworkers, and external partners · Ability to remain calm, professional, and effective during behavioral health crises and high-pressure situations · Ability to work effectively with individuals from diverse cultural and socioeconomic backgrounds while treating all persons with dignity and respect · Strong organizational, critical thinking, problem-solving, and time management skills · Ability to read and interpret laboratory results, healthcare documentation, safety procedures, policy manuals, and clinical records · Flexible and adaptable in a fast-paced and changing healthcare environment Physical Requirements · Perform sedentary work involving exertion of up to 10 pounds of force occasionally and negligible force frequently · Sit for extended periods with occasional standing and walking throughout the workday · Utilize repetitive hand, finger, and arm movements for typing, documentation, and clinical record review · Lift, carry, push, pull, or move light objects as necessary to Requirements
