About the role

The RN provides care for hospice patients through home visits, admissions, and symptom management. They are also responsible for handling after-hours phone triage and coordinating with a multidisciplinary care team.

Why Suncrest We are seeking a hopspice-experienced RN to work on-call.

This position works 7 days on, followed by 7 days off.

Schedule for the week working is Monday-Thursday from 4:30pm-8:00am, and from Friday at 4:30pm to Monday at 8:00am.

This position involves taking phone triage calls and seeing patients when needed throughout the greater St.

Louis area.

Actual Work/Life Balance Pay is $95,000-$105,000 per year + monthly bonus opportunities Competitive Pay Benefits Package including Medical, Dental, and Vision insurance 401k plan with employer match and 100% vesting after 90 days of employment A culture with an emphasis on appreciating and valuing the team member The opportunity to be part of a rapidly growing national company, with possible position upgrades Details The Registered Nurse (RN) will be responsible for providing care and services for a group of hospice patients, visiting patients where they reside, which could include private homes, assisted living facilities and hospitals. As a RN, you will complete hospice admissions and patient visits as needed. Applicants must have great clinical and communication skills. The RN is responsible for answering calls from patients, families and customers. The RN will make nursing visits for symptom management, deaths, and admissions as needed. This RN must also be passionate about hospice care and be a great advocate for both hospice services and Suncrest Hospice. This position will be after hours on-call working seven days/nights on and seven nights/days off.

Qualifications

Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice with one year of experience under license Admissions and On-call experience preferred Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership. Work closely with patients on a 1-1 level but get strong support from a patient care team. Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations. Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns) Ability to learn an electronic medical records system. Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance, to be able to travel across the service area

Requirements

Candidates must hold a current RN license in good standing with at least one year of experience. A valid driver's license, reliable transportation, and experience in hospice admissions or on-call services are preferred.
