The coordinator serves as the primary entry point for the Enhanced Care Management program, managing outreach, triage, and intake for eligible children and families. They are responsible for conducting comprehensive assessments, developing initial care plans, and building collaborative relationships with community partners.
(30%) Outreach and Community Engagement Develop and maintain collaborative relationships with Managed Care Plans, healthcare providers, behavioral health agencies, schools, child welfare agencies, probation, community-based organizations, and other referral partners. Conduct outreach activities in community settings, schools, clinics, shelters, and community events to identify and engage potential ECM participants. Educate community partners, families, and stakeholders regarding ECM services, eligibility requirements, referral pathways, and available resources. Coordinate outreach presentations, informational meetings, and stakeholder engagement activities. Create and distribute outreach and marketing materials consistent with agency and contract requirements. Participate in community collaboratives, networking events, and cross-system meetings and internal case conferencing meetings to strengthen referral pathways and partnerships. Promote culturally responsive and equitable access to ECM services. (35%) Triage, Assessment, and Intake Services Receive, review, and respond to referrals in accordance with ECM and Managed Care Plan requirements. Conduct triage screenings to determine eligibility, level of need, urgency, and appropriateness for ECM and other community services. Complete intake assessments to identify strengths, needs, risk factors, protective factors, and service priorities. Conduct assessments utilizing approved tools including, but not limited to, CANS, ANSA, risk assessments, crisis assessments, social determinants of health screenings, and other program- required instruments. Assess for immediate safety concerns and coordinate crisis intervention services when necessary. Gather and review relevant medical, behavioral health, educational, and social service information to support care planning. Complete intake documentation, informed consent forms, releases of information, enrollment packets, and required program records. Provide orientation to ECM services and explain participant rights, responsibilities, and program expectations. Collaborate with children, youth, families, caregivers, and referral sources to identify service goals and priorities. Utilize clinical judgment to determine service needs and make recommendations regarding services, resources, and enrollment. (20%) Enrollment, Care Planning, and Administrative Responsibilities Develop initial care plans based on assessment findings and identified needs. Facilitate enrollment into ECM services and coordinate warm handoffs to assigned Lead Care Managers. Coordinate referrals and linkage to healthcare, behavioral health, educational, housing, and community resources. Document all outreach, engagement, triage, intake assessment, enrollment, and care planning activities in accordance with Medi-Cal, CalAIM, Managed Care Plan, and agency requirements. Maintain accurate and timely documentation within the electronic health record. Participate in supervision, staff meetings, trainings, case consultations, and program development activities. Maintain accurate records and reporting related to outreach activities, referral sources, enrollments, and program outcomes. Ensure compliance with HIPAA, Medi-Cal, CalAIM, and agency documentation standards. Perform other duties as assigned. (15%) Program Coordination and Staff Support Serve as the primary coordinator for program outreach triage, intake, and engagement activities. Provide training, consultation, and technical assistance to Family Resource Center staff regarding ECM eligibility, referral pathways, intake procedures, and enrollment requirements. Coordinate with Family Resources Center leadership and staff to identify eligible participants and opportunities for outreach and engagement. Develop and implement standardized referral, screening, triage, intake processes across Family Resource Center programs. Facilitate communication and collaboration between Family Resource Center staff, ECM Lead Care Managers, Managed Care Plans, and community partners. Monitor implementation of activities and identify barriers to participants engagement, enrollment, and services access. Prepare reports and recommendations related to implementation, outreach effectiveness, and program outcomes. LOCATION: 1565 River Park Dr., Sacramento, CA 95815 SCHEDULE: Monday – Friday, 8:00am – 4:00pm or 9:00am – 5:00pm with occasional evenings and weekends based on business needs. Must be able to read, write, speak and understand the English language. Able to report to work on a regular and reliable basis. A valid CA driver’s license and minimum insurance coverage are required. Must be 21 years of age or older. Education and Experience: Highschool diploma or GED required. Master’s Degree from an accredited college or university in Social Work, Marriage and Family Therapy, Counseling, Psychology, Human Services, Public Health, or another behavioral health/healing arts field with demonstrated experience, prior to graduate school or through internships and practicum placements, conducting assessments and developing care plans and/or treatment plans and case management is preferred. OR A bachelor’s degree from an accredited college or university in Social Work, Human Services, Psychology, Sociology, Child Development, Counseling, Public Health, Behavioral Science, or a closely related field; and preference of a minimum of two years of professional experience conducting assessments, including evaluations of risk and protective factors, crisis assessment, needs assessment, CANS and development of care plans or treatment plans, experience providing field-based care coordination, case management, including engagement, assessment, service planning, resource linkage, and ongoing coordination with medical, behavioral health, and social service providers is preferred. OR Any equivalent combination of education, training, and experience that demonstrates the knowledge, skills, and abilities applicable to this position is preferred. Experience working with other systems such as behavioral health, child welfare, education, healthcare navigation, or community outreach services; working with children, youth, families, adults, and high-risk populations; outreach, community engagement, partnership development, and resource linkage is preferred. Licenses, Certifications, Registrations: CPR/First Aid certification is preferred.
