Position Summary

Clinical Case Manager Reproductive Health may assist with a variety of concerns impacting individuals and families, including but not limited to, the control and prevention of chronic conditions or infectious diseases, behavioral health conditions, and need for preventive services. Additionally,

Clinical Case Manager Reproductive Health services can help patients receive appropriate services related to perinatal care, preventive care, sexual and reproductive health, environmental and climate-sensitive health issues, oral health, aging, injury, and domestic violence and other violence prevention services.

Clinical Case Manager Reproductive Health tend to be members of the community they are serving and a larger component to linking health and social services for patients.

Clinical Case Manager Reproductive Health provide a broad range of services focused on improving the lives of individuals, families, and communities through providing meaningful, culturally relevant connections to healthcare and healthcare related services, which address the Social Determinants of Health (SDOH) on

par with the provision of general and specialty medical services. Clinical Case Manager Reproductive Health service is informed through the lived experience of the Clinical Case Manager Reproductive Health and their families, social circles, and communities.

Qualifications/Licensure

  • Bilingual English/Spanish (Required)
  • EHR- Electronic Health Record (Required)
  • Must be able to cope in a “fast pace” environment.
  • High School Diploma or GED
  • Comfortable working with diverse populations including formerly incarcerated individuals, undocumented individuals, LGBT populations, communities of color, and those experiencing homelessness, substance abuse, and/or mental illness.
  • At least 2 years’ experience conducting community and/or street outreach, specifically to: hard to reach populations.
  • Ability to drive and travel by car as needed.
  • Flexibility to travel between Los Angeles, San Bernardino, and Riverside Counties as needed.

Responsibilities

  • Provide case management services to patients in need of family planning, reproductive health and sexually transmitted infections (STI)
  • Screen patients, service planning and resource identification; linking clients to needed services
  • Work with interdisciplinary team to provide comprehensive family planning, reproductive health and sexually transmitted infections (STI) education and services.
  • Conduct patient, identification, assessments, diagnosis and interventions
  • Service implementation and coordination, monitoring service delivery, and advocacy
  • Conduct presentations and trainings to staff, patients, and community members in a variety of settings
  • Establish and develop working relationships with clients; initiate outreach to the target population using telephone, mail, and visits in the community.
  • Attend regularly scheduled meetings and maintain communication with program director and other staff members.
  • Maintain outreach activity calendar/Log (i.e., location, length of outreach event, encounters per event).
  • Assist with program evaluations and updates to program and services.
  • Attend appropriate community resource meetings and trainings assigned.
  • Create and implement an evaluation tool to measure the success of counseling and outreach efforts.
  • Conduct outreach activities and classes in the community.
  • Some evenings and Saturdays will require to work
  • Other duties may be assigned or may be modified as business needs dictate.
  • Conduct program evaluation.

Requirements

Requires a High School Diploma or GED, bilingual proficiency in English and Spanish, and at least 2 years of experience in community or street outreach. Candidates must be comfortable working with diverse, marginalized populations and have the ability to travel across multiple Southern California counties.
