Description Vertis Therapy is seeking a compassionate and dedicated PRN Speech Language Pathologist (SLP) for consistent hours at our beautiful assisted living facility, Commons at Jackson Creek in Bloomington Indiana. In this role, you will evaluate and treat residents to help improve their communication, cognitive, and swallowing abilities, independence, safety, and overall quality of life.
The Speech Language Pathologist will develop individualized treatment plans, provide skilled speech-language pathology services, monitor resident progress, and collaborate with the interdisciplinary team to support positive outcomes.
Resident Assessment and Treatment: Evaluate residents' communication, cognitive, and swallowing abilities. Establish and implement individualized treatment plans based on resident needs and goals to improve function, independence, and quality of life.
Education: Master's degree in Speech-Language Pathology from an accredited institution. Current Certificate of Clinical Competence (CCC) from the American Speech-Language-Hearing Association (ASHA) or be eligible for a Clinical Fellowship Year (CFY).
Valid Speech-Language Pathologist state license or license eligibility.
Computer proficiency and ability to document in an electronic medical record platform.
