Description Vertis Therapy is looking for a passionate Physical Therapist for consistent PRN hours at our beautiful Assisted Living, Commons at Jackson Creek, in Bloomington, IN. Vertis Therapy is a therapist-owned and operated company that specializes in providing patient-centered care.
The Physical Therapist will develop individualized treatment plans, provide skilled physical therapy services, monitor resident progress, and collaborate with the interdisciplinary team. The PT will also provide guidance and supervision to Physical Therapy Assistants (PTAs) as appropriate.
Resident Assessment and Treatment: Evaluate residents' functional abilities. Establish and execute a plan of care based on individual goals to improve resident's safety, functional mobility, and overall independence.
Complete timely and accurate documentation.
Provide appropriate guidance and supervision to Physical Therapy Assistants.
Professional
Development: Participate or lead team development meetings.
Wellness
Programming: Regularly lead community wide wellness programs and events.
Education: Degree in Physical Therapy from an accredited institution.
Valid Physical Therapist state license or license eligibility.
Computer proficiency to document in electronic medical record platform.
