Mental Health Clinician | Blackburn Center | Central City Concern

Make a lasting impact at one of Portland's most innovative programs focused on ending homelessness. The Blackburn Center brings together housing, primary care, behavioral health, substance use treatment, employment services, and recovery support under one roof, creating truly integrated, client-centered care for some of our community's most vulnerable populations. As a Mental Health Clinician, you'll join a multidisciplinary team providing trauma-informed, whole-person care to individuals experiencing complex mental health, substance use, medical, and housing challenges. You'll conduct assessments, develop treatment plans, provide individual and group counseling, support crisis intervention efforts, and collaborate closely with medical providers, case managers, housing teams, and community partners.

This role is ideal for clinicians who are passionate about serving underserved populations, believe in harm reduction and recovery-oriented care, and want to work in an environment that values collaboration, innovation, cultural responsiveness, and continuous improvement. Whether you're a QMHP, CSWA, LCSW, LPC, or LPC-A, you'll have the opportunity to build meaningful client relationships while helping create pathways to stability, wellness, and long-term housing success.

Schedule: Monday-Friday 7:30am-5:00pm (Saturday & Sunday off)

Salary Ranges from

$62,000- $124,000 *Final comp will be determined based on relevant experience, education, credentials/licensure, and internal equity.

Must meet qualifications for

  • QMHP or QMHP-R
  • LPC-A or LPC
  • CSWA or LCSW
  • CADC-R, at minimum, certification preferred, or the ability to obtain one within 1 year.
  • Has the competencies necessary to:
  • Identify precipitating events
  • Gather histories of mental, emotional and physical disabilities, alcohol and drug use, past mental health services and criminal justice contacts
  • Assess family, social and work relationships
  • Conduct a mental status assessment
  • Document a multiaxial DSM diagnosis
  • Write and implement or supervise implementation of a treatment plan
  • Conduct and document a mental health assessment
  • Provide mental health treatment and rehabilitative services within ones scope of practice
  • Must meet CCC privileging requirements as required by FTCA.
  • Must have current CPR certification prior to start.
  • Bi-lingual (English/Spanish) preferred.
  • Must become a certified Director's Custody designee in Multnomah County within the first 6 months of hire.
  • If using a vehicle for agency use: Must possess a current driver's license, access to a vehicle, qualify as an Acceptable Driver as designated in Central City Concern's Fleet Safety policy; pass an initial drivers training within 60 days of being an approved driver and continued recertification training; and maintain vehicle insurance coverage of a minimum of $100,000/$300,000 personal auto liability coverage.
  • Must pass a pre-employment drug screen, TB Test, and background check. This includes clearance by the DHS Background Check Unit.
  • Ability to adhere to Central City Concern’s drug-free workplace which encourages a safe, healthy, and productive work environment and strictly complies with the Drug-Free Workplace Act of 1988. An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess, or use a controlled substance or alcohol.
  • Must generally have the ability to walk, bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects, and climb stairs with reasonable or no accommodation.
  • Must adhere to agency's non-discrimination policies.
  • Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, political affiliation, cultural backgrounds, life-styles and sexual orientations, and treat individuals with respect and dignity.

Essential duties and responsibilities

  • Work as a member of the Blackburn Center multi-disciplinary team to deliver comprehensive, client centered, and trauma informed integrated services.
  • Actively participate in the integration of services at the Blackburn Center and identify new opportunities to improve collaboration between and within service and care teams while carefully following information-sharing guidelines.
  • Proactively identify opportunities for improvement within Blackburn Center workflows and logistics. Participate in review of Blackburn Key Performance Indicators; review and contribute to continuous improvement opportunities and plans.
  • Build understanding of and appreciation for diversity among Blackburn Center clients and support their connection to culturally responsive and culturally specific services as requested.
  • Collect information regarding clients' history through a combination of interviewing, records review, and consultation, after obtaining written consent.
  • Conduct and document Mental Health Assessments, including mental status exam, DSM diagnosis, and clinical formulation.
  • Develop care plan with client with assistance & input from all of the clients care team providers
  • Participate in outreach activities necessary to offer services and engage participation in a non­-coercive manner. Escort clients to meetings and appointments as necessary.
  • Participate in evaluating clients for risk of harm & initiate Directors Custody transport holds when appropriate.
  • Participate in clinical staff meetings and case conferences with team members in order to utilize interdisciplinary expertise and facilitate individual treatment plans for clients;
  • Collaborate with Residential facilities, Adult Foster homes or other housing providers to provide necessary treatment services and coordinate residential and non-residential treatment and service planning.
  • Participate in admission and discharge planning for any client needing hospitalization or subacute level of care. Client contact must be made within one working day of admission.
  • Monitor and assure that the health and safety needs of clients are met.
  • Maintain client records using an electronic medical record. Must be documented as required by program standards and funding requirements including documentation of clinical interactions, behavioral health assessments & individual service plans.
  • Participate in regular supervision from the Clinical Supervisor, seeking additional supervision whenever necessary. Minimum requirement two hours of documented supervision per month.
  • Attend all mandatory CCC trainings in a timely manner.
