The Hospice RN Case Manager manages an assigned patient caseload by assessing, planning, and implementing care plans in a home-based setting. They also collaborate with an interdisciplinary team to provide support to patients and their families throughout the end-of-life process.

Purpose: Ennoble Care is hiring for a full-time Hospice RN Case Manager who resides within or in close proximity to Chester County, PA.

As a member of the interdisciplinary team, the RN Case Manager works under the general direction of the Director of Nursing, the attending physician and the interdisciplinary group in providing skilled nursing assessments, planning, and care in order to maximize the comfort and health of both patients and their loved ones/caregivers.

What You'll Do: Manage your assigned patient caseload within your territory and be responsible for the following tasks, including but not limited to: assessing, planning, implementing, and evaluating of patients care throughout all phases of the nursing process.

Document all notes in the EMR and submit them accurately and in a timely manner Communicate and partner cross-departmentally with all attending physicians, hospice staff members, and other agencies as needed to coordinate optimal care for all patients Educate and coach all relevant parties on the best practices regarding call levels of care for the patient including: physical, mental, emotional, etc Partner with clinical leadership to manage/address any issues, share professional support, and exchange feedback to enhance professional growth.

Attend patient care conferences and facilitate discussion of issues from caseload for full staff discussion, consultation and evaluation Supervise and document the NAC every two weeks or as assigned Assume the role of associate nurse when responding to patient/loved ones/caregivers of other primary nurses during off hours time or as requested by the Patient Care Coordinator to fill patient/family needs during the absence of other primary nurses Provide appropriate support at time of death and period of bereavement for family members and loved ones Participate in the Hospice orientation, in-service training, and community programs as related to hospice care Maintain familiarity with policies of the agency and rules and regulations of State and Federal bodies which govern hospice Accept other assignments as appropriate What You'll Need: Valid State license to practice as a Registered Nurse in the State of Pennsylvania 1-2+ years of work experience as a professional nurse preferably at least one of the years in hospice care Ability to assess and respond to the needs of patients and loved ones/caregivers in varied settings.

Skilled in nursing practice, able to cope with loved ones/caregivers’ emotional stress and tolerant of individual lifestyles Ability to make meaningful observations, skilled assessments, and write comprehensive, accurate reports Must be a team player who supports an interdisciplinary team concept of caring for patient and loved ones/caregivers Must have a valid driver’s license, a good driving record, the use of a reliable vehicle, and the ability to drive year-round to locations Must be able to take direction, work independently and effectively as part of a multidisciplinary team in a positive, professional manner which supports the mission of the organization What You'll Get: A flexible schedule - so you can see your patients on a schedule that works for you Autonomy - so you can feel trusted to provide the best care possible Support - from our in house clinical team with any requests Growth - the opportunity to step into leadership positions Mileage Reimbursement Pay Range: Starting at $90,000 salary #red Full-time employees qualify for the following benefits: Medical, Dental, Vision and supplementary benefits such as Life Insurance, Short Term and Long Term Disability, Flexible Spending Accounts for Medical and Dependent Care, Accident, Critical Illness, and Hospital Indemnity.

Requirements

Candidates must hold a valid Registered Nurse license in Pennsylvania and have at least 1-2 years of professional nursing experience, preferably in hospice. A valid driver's license and reliable vehicle are required for travel to patient homes.
