High-Fidelity Wraparound is an evidence-based, intensive care coordination model to support children and youth with serious behavioral health challenges in living successfully in the community with their families.
All members served by the Hi-Fidelity Wraparound Care Manager are enrolled in the NYS Department of Health Children’s Health Home Care Management (Community Outreach & Care Coordination) program.
The High-Fidelity Wraparound Care Manager’s primary responsibility is to assist in developing a cohesive Child and Family Team composed of the different people who provide services and support to the member and their family.
Child and Family Team members may include healthcare, social care needs providers, school personnel, and the family’s natural supports.
The Child and Family Team is responsible for creating and carrying out an integrated and comprehensive Wraparound Plan grounded in the youth and family’s individualized needs and strengths.
It is strongly informed by their cultural values, choices, and vision for the future.
Hi-Fidelity Wraparound Care Manager is responsible for ensuring the model is implemented with fidelity for each member and family while also carrying out all elements of the Children’s Health Home Care Management (Community Outreach & Care Coordination) program.
We run a thorough and complete pre-employment background check on all candidates being considered for a position, this includes fingerprinting. All candidates must successfully clear the pre-employment background check process, which follows NYS/DOJ requirements. We pay for all pre-employment background checks!
