The Home Health Nurse provides high-acuity nursing care, including wound care and infusions, directly in patients' homes. They are responsible for conducting physical assessments, developing individualized care plans, and educating patients and families to support healing.

Job Summary and Responsibilities As our Home Health Nurse, you will be a pivotal healthcare hero, bridging hospital care with the comfort of home, empowering patients to heal in their own environment.Every day, you will deliver personalized, high-acuity nursing care for a diverse range of conditions and ages directly in patients' homes.

Completes an initial assessment of client and family to determine home health needs.
Provides a complete physical assessment and history of current and previous illness.
Regularly re-evaluates client nursing needs.
Initiates the plan of care and makes necessary revisions as client status and needs change.
Uses health assessment data to determine nursing diagnosis.
Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
Includes the client and the family in the planning process.
Initiates appropriate preventive and rehabilitative nursing procedures.
Administers medications and treatments as prescribed by the physician.
Counsels the client and family in meeting nursing and related needs.

Job Requirements Required Graduate of National League for Nursing accredited school of nursing.
Experience in the area of public health or home health nursing Registered Nurse: ND Basic Life Support - CPR Driver's License Auto Insurance
Candidates must be graduates of an accredited school of nursing and hold a valid Registered Nurse license. Experience in public health or home health nursing is required, along with BLS certification and a valid driver's license.
