The in patient hospice nurse acts as an end of life specialist and highly skilled resource related to the general in patient level of care. The RN will take the lead to assess, plan, implement, coordinate, monitor and evaluate options to facilitate the continuum of care through discharge planning. The RN collaborates with the patient or patient’s family/representative, the physician, and members of the multidisciplinary team to facilitate progression-of-care; intervening appropriately to ensure that the plan of care and services provided are patient focused, high quality, evidence based, appropriate to patient needs, efficient, and cost effective. The nurse serves as a resource to the physicians and provides education and information on resource utilization, national and local coverage determinations, and use of hospital admission criteria. This role requires a high-level of leadership, interpersonal skills, and hospice knowledge. Education Required: Graduate of an accredited school of nursing Preferred: Bachelor's of Nursing Experience Required: Minimum of 1 year hospital experience Preferred: Minimum of 1 year hospice experience Skills Excellent interpersonal skills, including ability to be genuine and empathetic Excellent communication (verbal and written) skills with attention to detail Computer skills Ability to interact professionally and contribute to the interdisciplinary team Licensure/Certification/Registration Current RN License in the state of Missouri Preferred: Certified Case Manager
Candidates must be a graduate of an accredited school of nursing and hold a current RN license in the state of Missouri. A minimum of one year of hospital experience is required, with hospice experience preferred.
