About the role

The Nursing Assistant provides personal care, comfort, and homemaker services to terminally ill patients in their homes or the hospice facility. They also monitor patient conditions, report changes to the registered nurse, and assist with daily living activities.

Job Summary To provide personal care and comfort services to terminally ill patients, regardless of their age, and perform related homemaker tasks in the patient/family residences or Cottingham Hospice House, in accordance with the plan of care, as assigned and supervised by the registered nurse.

Accountabilities 1.

Personal care and comfort.

The CNA must be proficient in the completion and documentation of the following tasks: a.

Bathes patient in the bed, chair, tub or shower providing for patient's privacy, comfort and safety while doing so b.

Assists /provides mouth care c.

Shampoos, styles and/or combs patient's hair d.

Performs basic nail care and foot care e.

Shaves patient f.

Assists patient to dress g.

Assists with ambulation, transfer, use of assistive devices h.

Turns and positions patient i.

Assists patient to perform range of motion exercises j.

Facilitates elimination, assisting patient to use BSC, diapers, bedpan, urinal or to care for indwelling catheter or ostomy appliance k.

Facilitates nutrition, assists by preparing meal, feeding patient and/or by offering fluids l.

Models competent personal care techniques and communication skills for the caregiver m.

Performs all of the above tasks for all ages/developmental stages n.

Participates in bereavement care by attending visitations/funerals - 30% 2.

Monitor Patient's condition The CNA will assist the nurse by monitoring the patient's condition and reporting any changes noted to the nurse promptly.

Measure and record vital signs ( temperature, pulse, respiration, blood pressure, weight) as designated in POC. a.

Measures I&O b.

Observes and reports changes in skin color, reports any skin tears c.

Monitors the condition of non-sterile dressings and with the direction of the RN changes the dressing when needed. d.

Reports any difficulties with elimination e.

Reports any changes in patient's mental status i.e. ability to communicate, level of confusion or any aggressive/agitated behavior f.

Effectively utilizes communication skills /listening skills to promote patient comfort with the procedure.

Encourages patient to express needs or preferences in personal care g.

Observes and reports patient compliance with safety measures h.

Participates in team conference and IDG as requested to discuss patient and caregiving issues i.

Reports any changes in patient's physical strength, endurance or coordination that would alter the delivery of care as assigned on the CNA POC j.

Is knowledgeable in the issues related to death and the normal grieving process and is able to be supportive of patient and family. – 30% 3.

Homemaker Tasks The CNA performs basic housekeeping duties, as assigned, that promotes hygiene, comfort and safety for the patient and enables the patient to remain in the home or the Cottingham House. a.

Changes linen and bed clothes; Launders linen, towels and clothing (in home) b.

Washes dishes and utensils.

Cleans and sanitizes kitchen counters c.

Empties out of date food from refrigerator d.

Organizes food and supplies.

Delivers medical supplies as requested e.

Works with caregiver to remove unnecessary clutter from patient's environment in order to reduce risk of falls f.

Cleans and sanitizes durable medical equipment g.

Cleans and sanitizes bathroom area h.

Runs errands for caregiver/patient as assigned by RN - 30% 4.

Agency Needs - The CNA performs the following tasks in support of the general needs of the department: Is thoroughly familiar with dept. policies and procedures and demonstrates compliance with hospice standards of care, including infection control and bloodborne pathogens.

Participates in review of QI data and makes appropriate recommendations for improvement Manages the agency resources by demonstrating reasonable and efficient use of time, mileage visit schedules and pre-visit activity.

Monitors utilization of medical supplies, maintains adequte supply in the home to meet patient's needs Assists in organizing, cleaning storeroom Assists with office duties ( i.e. filing, telephone, errands) as requested – 10% Supervisory/Management Responsibilities This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements Job related course work beyond high school - completion of a nursing assistant or patient care tech program Required Certifications, Registrations, Licenses CNA - NURSING ASSISTANT - NCNA SCDL - SC DRIVERS LICENSE - LSCD Knowledge, Skills or Abilities Basic computer skills including word processing and data entry Knowledge of office equipment (fax/copier) Mathematical skills Proof of auto liability coverage Must be willing to travel and have reliable personal transportation Work Shift Day (United States of America) Location Hospice Of The Foothills Facility 1067 Hospice - Upstate Department 10687404 Nursing Aides Share your talent with us!

Requirements

Candidates must have completed a nursing assistant or patient care technician program and hold a valid CNA certification. A valid South Carolina driver's license and reliable personal transportation are also required.
