The LPN Patient Services Coordinator is responsible for scheduling patients with field clinicians and facilitating communication between staff, patients, and physicians. They also manage on-call notebooks and hospitalization logs to ensure continuity of care.

Responsibilities

Schedule patients to branch field clinicians. Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers in order to maintain proper care coordination and continuity of care. Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.

Qualifications

  • Must possess a high school diploma or equivalent. Must be a licensed practical or vocational nurse in the state in which they currently practice, with at least one year of clinical experience in a healthcare setting Must have basic demonstrated technology skills, including operation of a mobile device. Education and experience, preferred Previous experience in home health, hospice, or pediatrics is preferred. Requirements
  • Must possess a valid state driver license Must maintain automobile liability insurance as required by law Must maintain dependable transportation in good working condition Must be able to safely drive an automobile in all types of weather conditions.

Requirements

Candidates must hold a high school diploma and a valid LPN or LVN license with at least one year of clinical experience. A valid driver's license, reliable transportation, and automobile insurance are required for this role.
