The Occupational Therapist will perform home visits to evaluate patient functional status and develop individualized intervention plans to improve independence. They will also educate patients and families on rehabilitative care and document all services provided using electronic medical records.

Candidates must hold a current NJ Occupational Therapist license and have at least one year of recent work experience. Graduation from an accredited occupational therapy program is required, and home care experience is preferred.
