  • Purpose: - The Care Coordinator at LIFESTREAM BEHAVIORAL CENTER is responsible for ensuring seamless care delivery to clients by coordinating various aspects of their treatment plans. This role involves working collaboratively with healthcare providers, clients, and their families to facilitate access to necessary services and resources, ultimately enhancing the quality of care and improving client outcomes.

  • Responsibilities: - Develop and implement individualized care plans in collaboration with healthcare providers and clients. - Coordinate and schedule appointments, follow-ups, and necessary treatments for clients. - Serve as a liaison between clients, families, and healthcare professionals to ensure clear communication and understanding of treatment plans. - Monitor client progress and adjust care plans as needed to address changing needs and circumstances. - Facilitate access to community resources and support services to aid in the client's recovery and well-being. - Maintain accurate and up-to-date documentation of client interactions, care plans, and progress notes. - Advocate for clients' needs and preferences within the healthcare system to ensure they receive appropriate and timely care. - Participate in multidisciplinary team meetings to discuss client cases and develop comprehensive care strategies. - Educate clients and their families about the treatment process, available resources, and self-care strategies. - Ensure compliance with all relevant regulations, policies, and procedures in the delivery of care services.

  • Education: - Bachelor's degree in Social Work, Psychology, Nursing, or a related field.

  • Experience: - Minimum of 2 years of experience in a healthcare or behavioral health setting. - Demonstrated experience in case management or care coordination. - Experience working with diverse populations, including individuals with mental health and substance use disorders.
