  • Setting: Outpatient Family Medicine Clinic Position Overview We are seeking a full-time Nurse Practitioner or Physician Assistant to join our community-focused outpatient family medicine clinic in Dewey-Humboldt, AZ. This is an excellent opportunity for providers who value long-term patient relationships, a supportive team culture, and a predictable weekday schedule. With no nights, weekends, or on-call duties, this role allows you to focus on high-quality, patient-centered care. We focus on whole-person care, provider well-being, and long-term community health outcomes, offering a supportive structure that allows you to practice at the top of your license. Key Responsibilities Perform annual wellness exams, preventive screenings, and Medicare visits Diagnose and manage acute conditions and minor injuries Provide chronic disease management (diabetes, hypertension, COPD, etc.) Order, interpret, and follow up on diagnostic tests and imaging Educate patients on medications, treatment plans, and healthy lifestyle practices Maintain accurate EHR documentation with synchronous scribe support Coordinate referrals and manage transitions of care Participate in clinical quality initiatives and practice improvement efforts

Qualifications

Active and unrestricted Arizona NP or PA license Current DEA registration or ability to obtain Credentialing eligibility with Medicare, Medicaid, and commercial insurers No history of malpractice or disciplinary action Graduate of an accredited U.S.-based NP or PA program (Note: Puerto Rico-based programs are not accepted) Who Thrives Here A team-oriented, adaptable, and compassionate clinician who enjoys proactive primary care and building meaningful patient relationships in a community setting.
