About the role

Provide direct patient care including assistance with activities of daily living, mobility, and personal hygiene. Monitor and record vital signs while reporting any changes in patient condition to nursing staff.

LNA Skilled Nursing Facilities New Hampshire - Lebanon, Franklin, Claremont, Concord, Milford, Keene, Rochester, Exeter Contract Days/Mid/Nights $18-$21/hr

Duties & Responsibilities

Personal Hygiene & ADLs: Help residents with bathing, dressing, grooming, oral care, and toileting or incontinence care. Mobility & Transfers: Assist residents with walking, transferring between beds and wheelchairs, and repositioning bedbound residents to prevent pressure sores. Meals & Hydration: Serve meal trays, feed residents who need assistance, and track fluid and food intake. Vital Signs & Monitoring: Measure and record blood pressure, pulse, temperature, weight, and daily output (urine/bowel movements). Reporting & Observation: Alert LPNs or RNs immediately about changes in a resident’s physical condition, skin breakdown, mood, or behavior. Comfort & Safety: Keep resident rooms clean and safe, answer call lights promptly, change bed linens, and provide emotional support. Active LNA license CPR/BLS (if available) 1 year of experience

Requires an active LNA license and at least one year of experience. CPR/BLS certification is preferred if available.
