• Bonus: $6,000 JOB SUMMARY The Licensed Practical Nurse (LPN) is a member of the PACE interdisciplinary team; provides nursing care to the participant and assists the Registered Nurse with the implementation of the Plan of Care. The LPN provides care for the participant in a manner that promotes an interdisciplinary team approach, respects participant’s goals for care, meets Medicare/Medicaid conditions of participation and accreditation requirements and reflects the mission and philosophy of TRU PACE. Participates in all areas of clinical services. EDUCATION AND QUALIFICATIONS Must possess a current LPN license in the state of Colorado Minimum one year LPN experience – hospice or home health experience preferred Experience working with frail/elderly preferred Experience working with individuals with mental health needs desirable Bilingual in Spanish preferred Must successfully pass agency required background checks and drug screening IV certification and phlebotomy skills preferred ESSENTIAL DUTIES Collects data regarding all body systems, signs and symptoms, deviations, acuity, vital signs, and responses in collaboration with the Registered Nurse and primary care physician, Incorporates participant preferences and goals within the plan of care Supports and respects the participant and caregiver’s unique diversity, including spiritual and cultural beliefs. Completes documentation in an accurate and timely manner as per agency standard and in compliance with regulatory standards Provides participant and caregiver education as appropriate; areas of education include, but are not limited to personal care, medications, procedures, safety, diet and nutrition, and health and wellness through the lifespan
