The Case Manager serves as a primary point of contact for families, managing caseloads and developing individualized service plans for individuals with Autism Spectrum Disorder. They also coordinate referrals, collaborate with multidisciplinary teams, and conduct community outreach and educational initiatives.
Description Pay rate will be between $22.00 to $25.00 per hour (and based on experience in that range)

Easterseals is seeking a dedicated and compassionate Case Manager – Autism Services to support individuals with Autism Spectrum Disorder (ASD) and their families. This role serves as a key advocate, liaison, and educator, helping clients navigate the continuum of care from screening and assessment through service coordination and discharge.
The Case Manager plays a critical role in ensuring families have access to the resources, education, and support needed to achieve positive outcomes.
