Summary Provides clinical support and assists in the coordination and delivery of patient care services during or after normal business hours. Performs data/order entry, clinical record reviews, and other clinical tasks related to the provision of patient care. Plans, organizes, and directs care in collaboration with the interdisciplinary team to meet the needs of the patient and family in their homes and communities.
Job Duties Performs telephone triage for patients calling during or after hours utilizing critical thinking skills and sound clinical judgement Performs assessments of patient status, provides direct nursing care, and addresses immediate needs related to physical, psychosocial, and environmental factors. Provides educational opportunities for patients, families, and clinical staff focusing on end-of-life issues, palliative care, advance directives, pain management, symptom control, home care, hospice, and discharge planning. Performs data/order entry, clinical record reviews and other clinical tasks related to the provision of patient care. Provides hand-off communication to RN covering next shift, interdisciplinary team members, and/or Clinical Manager as appropriate regarding triage information, patient status updates, and patients who require a follow up visit or phone call.
Specialized Diploma NLN approved/accredited nursing program 1 year recent experience providing direct care in a healthcare setting Demonstrates knowledge and ability to care for patients based on the population served. Physical Demands Lift and carry 40-50 lbs.
Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
