Summary We are seeking a dependable and compassionate Healthcare Coordinator to support participants in accessing and navigating medical care. In this hands-on role, you will serve as a key liaison between participants, healthcare providers, and internal nursing staff to ensure coordinated and effective care. Reporting to Health Services leadership, you will spend your time transporting participants to appointments, supporting them during medical visits, and ensuring accurate communication and documentation across care teams. This role offers a meaningful opportunity to directly impact participant health outcomes through advocacy and coordination.

The Role Provide advocacy and support for participants during medical appointments, procedures, and healthcare interactions Transport participants safely to and from medical appointments throughout the local area Serve as a liaison between participants, healthcare providers, and internal nursing staff to ensure coordinated care Build and maintain positive relationships with community healthcare providers Advocate effectively for participant needs during routine care and emergency situations Maintain accurate and timely health documentation to support communication across care teams Support participants with mobility needs during transportation and appointments as required Follow infection control protocols and safety procedures when supporting individuals with varying health conditions Collaborate with nursing staff and team members to ensure continuity and quality of care

Qualifications

High school diploma or equivalent Valid driver's license from Illinois, Indiana, or Wisconsin with a clean driving record Experience supporting individuals in healthcare settings or navigating medical systems preferred Ability to communicate effectively with medical professionals, participants, and team members Strong organizational skills and attention to detail, particularly in documentation Ability to remain calm and supportive in potentially stressful or emergency situations Willingness to provide care involving exposure to illness while following proper safety protocols Physical ability to assist participants with mobility needs during transportation and appointments Basic computer skills for documentation and communication Compensation The hourly rate for this role is $19.00 per hour
