This position is needed for enabling provider productivity at the HOME Program. It is responsible for clinical procedures, patient intake and rooming, medication and immunizations, lab duties, patient education and care coordination. This position is 100% grant funded.
Provide hands-on care under RN/physician supervision, including administering medications, monitoring vitals, wound care, triaging/rooming patients within 15 minutes, and performing treatments. Accurately document patient info, vitals, interventions, and medication administration with 100% compliance. Collect, label, and log specimens (blood, biopsy) with zero tolerance for error. Clean and stock exam rooms between sessions, maintain supply par levels, and sterilize equipment via autoclave. Partner with the healthcare team to escalate issues, facilitate discharge, and teach patients/families regarding care management. Provides direct care for a specific patient population. Administers medications and treatments. Provides patient & family teaching and discharge instructions. Required NY LPN License & Registration BLS Card and Infection Control Certification High School/GED minimum Shift: Monday to Friday 0900A – 0530P; Saturday rotation with weekday off; Occasional late shift of 1030A – 0700P or 1100A – 0730P Montefiore Medical Center’s advancement in the healthcare industry is also yours.
