Responsibilities

Providing medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician Completing history and physical examinations by evaluating the patient's medical condition and health history Completing the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based Making recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care Completing and maintaining accurate clinical notes Communicating with the physician and participating in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family/caregivers Ensuring compliance with all state and federal legal and regulatory requirements Additional responsibilities as assigned

Qualifications

Graduate from an accredited School of Nursing and accredited Nurse Practitioner program Current DEA license, if applicable to state regulations Current nursing license in state of practice Current CPR certification Master’s degree with minimum of one year NP experience One year hospice or palliative care experience preferred Accreditation from an approved certifying body for advanced practice nursing as required by state of practice Certification in a specialist area preferred (e.g. Hospice and Palliative Nursing (CHPN) or Geriatrics) Commitment to clinical and documentation excellence About our Line of Business At Haven Hospice, our primary ambition is to bring high-quality care to the warmest and most serene environment possible: home. Requirements

Candidates must hold a Master's degree, a current nursing license, and a DEA license. At least one year of experience in a Nurse Practitioner role and hospice or palliative care is required.
