About the Role

Bring your nursing expertise to one of healthcare's most meaningful specialties. As a Hospice RN Case Manager, you'll provide skilled nursing care and compassionate support to patients and families during life's final chapter, helping ensure comfort, dignity, and quality of life. This role is perfect for a registered nurse who wants to practice the full scope of nursing while building deep, meaningful relationships. This position will have you building report with patients, families, physicians, facilities, and other interdisciplinary team members. You'll have the autonomy to coordinate comprehensive care for your caseload while working collaboratively with our interdisciplinary team to address the physical, emotional, and spiritual needs of those in your care. Location Office Location: Saginaw, MI Main Service Area: Saginaw, Clare, Gratiot, Isabella County and surrounding areas. This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes Hours Monday to Friday, 8 a.m. to 5 p.m., with weekend rotation Required Qualification Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing Current State License as a Registered Nurse Minimum one (1) year experience as a nurse in a clinical care setting Reliable transportation, valid driver’s license, and valid automobile insurance coverage

Requirements

Candidates must hold a current State License as a Registered Nurse and have at least one year of clinical nursing experience. A degree from an accredited Diploma, Associate, or Baccalaureate School of Nursing is required, along with reliable transportation and a valid driver's license.
