As a community health worker (CHW), you’ll have an important role in creating a positive experience for patients at Neighborhood.
You’ll listen with kindness to patients’ concerns, seek solutions, and assist patients and families with navigating services and community resources. Responsibilities include:
Consults with providers regarding patient needs for referrals or linkages and assist patients in accessing and completing referrals.
Assists patients in accessing community resources such as food pantries, clothing, shelter, transportation, education, parenting or other services as referred by providers.
Assist patients in completing healthcare forms such as applications for Medicaid, Child and Family Health Plus and Sliding Fee Scale program. Connect patients to facilitated enrollers.
Obtains prior authorizations as needed. Documents and tracks referrals until completion.
Conducts community outreach to identify people who are experiencing homelessness and educates them about health needs.
Supports people who are experiencing homelessness in accessing healthcare services at Neighborhood.
Serves as health and community resources coordinator for patients and providers.
Advocates for patients with community agencies and services.
Conducts outreach and speaks to community groups.
Assists in developing Neighborhood staff to improve patient navigation, decrease barriers for patients in accessing care and connect patients to care.
You will be primarily based at our Blasdell location, 4233 Lake Avenue, Blasdell.

30 a.m. and 4:30 p.m., and have ability to travel for community outreach and work extended hours once a week 7:00-9:00 p.m.
$22.25 an hour based on a full-time, 40-hour workweek
