Are you a compassionate and skilled Nurse Practitioner with a passion for cardiological care? Monadnock Community Hospital (MCH) is seeking a Cardiology Nurse Practitioner to join our dedicated team of healthcare professionals committed to delivering exceptional patient-centered care.

This is an exciting opportunity to work collaboratively with cardiologists, physicians, and multidisciplinary healthcare teams while helping patients manage acute and chronic cardiological conditions. You will have the autonomy to practice at the top of your license while making a meaningful impact on the health and well-being of our community.

What You'll Do

As a Cardiology Nurse Practitioner, you will

Perform comprehensive cardiological assessments, including detailed patient histories and physical examinations.

Evaluate, diagnose, and manage patients with a wide range of cardiac conditions.

Order and interpret laboratory, diagnostic, and imaging studies.

Develop individualized treatment plans and evidence-based care strategies.

Prescribe medications and therapies in accordance with state regulations and clinical guidelines.

Collaborate closely with physicians and other healthcare professionals to optimize patient outcomes.

Monitor patients between visits and ensure continuity of care.

Triage patient calls and provide clinical consultation as appropriate.

Educate patients and families on cardiac disease prevention, treatment plans, lifestyle modifications, and wellness strategies.

Promote heart health through community outreach, education programs, and employer wellness initiatives.

Maintain accurate, timely, and compliant clinical documentation.

Support quality improvement initiatives and contribute to a culture of excellence, safety, and patient-centered care.

Qualifications

  • Required Education & Licensure
  • Graduate of an accredited School of Nursing.
  • Successful completion of an accredited Nurse Practitioner program.
  • Current New Hampshire APRN license.
  • Current DEA registration.
  • Ability to obtain and maintain professional liability insurability through MCH.
  • Preferred Experience
  • One or more years of experience in a clinical practice setting.
  • Cardiology, internal medicine, primary care, or related specialty experience preferred.

Requirements
